A-Vision Community Health Services (AVCHS) is a nonprofit community-based organization committed to improving the health, wellness, and quality of life of senior citizens and vulnerable populations through culturally grounded Community Health Worker (CHW) programming.

Founded on the belief that wellness begins with connection, AVCHS provides accessible, compassionate care through outreach, education, and prevention-focused initiatives. Our programs emphasize health equity, intergenerational engagement, and the preservation of community legacy through storytelling, arts, and wellness.

What We’re Changing:

We reduce isolation by building meaningful connections through regular home visits, wellness checks, and social engagement.
We prevent avoidable health crises by providing early interventions, education, and linking seniors to trusted resources and providers.
We empower seniors to take control of their health, advocate for their needs, and remain active and engaged in their communities.
We support family caregivers by offering guidance, emotional support, and community resources.
We inspire community action by connecting volunteers, partners, and neighbors who believe in uplifting our elder population.

Our Goals:

Ensure that every senior we serve feels seen, valued, and supported.
Provide accessible health education to help seniors better manage chronic conditions and improve their overall well-being.
Expand our reach to serve more seniors across Richmond and surrounding areas.
Build a network of dedicated volunteers and partners who share our vision for a healthier, more connected senior community.
Advocate for systemic changes that improve access, dignity, and services for aging adults.
